The following are San Benito LAFCO's most frequently used application forms for boundary changes. These forms may be completed online, downloaded, and printed. Refer to the "Application Fees" document listed below to determine corresponding processing fees.
If a General Application Form is approved, the proponent must prepare a set of legal maps and legal descriptions for filing with the State Board of Equalization which requires an additional fee to finalize the boundary change.

To request a change of organization (i.e. an annexation), complete the following three forms:
1. General Application Form (required for all projects)
State law requires a signed application from individuals or agencies who want to apply for a boundary change cush as an annexation, extraterritorial service agreement, or formation of a new special district.
2. Notice of Intent to Circulate a Petition
State law requires this notice to be filed with the LAFCO Executive Officer before ciruclating a propertey owner or registered voter petition to initiate a boundary change. (Fill out if applicant is a landowner or registered voter)
3. Petition Form
State law provides that a General Application Form be submitted to LAFCO either by resolution of an affected public agency board or by resolution of registered voters or property owners. This petition can be used by a group of either registered voters or property owners to accompany a General Application Form. (Fill out if applicant is a landowner or registered voters)

State Board of Equalization's Change of Jurisdictional Boundary Requirements
At the end of a successful boundary change process, the applicant is required to provide a legal map and a legal description. A processing fee must also be paid to the State Board of Equalization to finalize the change. See requirements, samples, and fees below.
State CEQA Document Fees
The cost for LAFCO to process an application includes CEQA document fees. These fees are required by the state for many projects. LAFCO pays these fees which are charged back to the applicant.
